Unionville Crew Athletes and Parents,

Please complete this form for all planned absences from practice/regattas.  For unplanned absences (notification the week of an absence) completion of this form is required as well as speaking directly to your coach to notify him/her of the absence so that the appropriate line-up change may be made.

If you need to submit absences for multiple non-consecutive days please submit each date individually. Just wait for the page to reload and submit another form.
